Job Summary:

The University of Colorado, College of Nursing, is seeking a Family or Adult Gerontology Nurse Practitioner to serve as the Clinical Director for a full-time (1.0 FTE) benefits-eligible position with the University of Colorado, College of Nursing, at Sheridan Health Services (SHS). SHS has three sites, the CU Family Health Clinic in Denver, CO (80236), the Youth Health Clinic in Englewood (80110), and the CU Belleview Point Clinic in Aurora (80015). This position is clinic based as a primary care provider involving direct patient care (50% effort) and includes dedicated effort for administrator duties (50% effort). Additionally, this position will be appointed to a faculty position in the clinical practice track within the College of Nursing. Faculty rank is based on experience and qualifications.

The Clinical Director serves as the primary care advisor to SHS management and represents the interests of patients and primary care staff. The Clinical Director serves to define and explain clinical goals and directives to the medical staff. The Clinical Director is involved in efforts to ensure clinicians comply with regulatory and legal requirements,and directs efforts at maintaining and improving the quality of care provided to the SHS patient populations. The Clinical Director will serve as an effective leader of clinicians to ensure high quality, productive medical and nursing care.

Clinic Description:

SHS is a multi-site, nurse-managed FQHC, funded through the Health Resources and Services Administration Bureau of Primary Healthcare; and is a collaborative effort between the College of Nursing and the Sheridan School District. There are now 80 employees at three convenient locations to serve community health needs primarily for residents of Arapahoe, Denver, Adams, and Jefferson counties. The CU Youth Health Clinic is located in the Sheridan School District Administration building in Englewood, CO. The CU Belleview Point Health Clinic is located in southwest Aurora and the CU Family Health Clinic is located in southwest Denver on the Fort Logan campus and provides services to adults and children, 65% of whom identify as Latinx, and 42% of whom are best served in another language, with Spanish being the predominant second language. Services offered include primary care across the lifespan; prenatal and postpartum, gynecology, dentistry, pharmacy, behavioral health, and outpatient substance abuse treatment. SHS is a Patient-Centered Medical Home, and as such, we uphold a model of care that places the patient at the center, incorporating the values and beliefs of the patient in all decision-making. Examples of Work Performed:

Leadership:
  • Oversees leads and key staff for medical service lines, to include but not limited to Adult Primary Care, Pediatric Primary Care, OB & Psychiatry.
  • Oversees clinical initiatives and workflows for medical service lines, to include but not limited to Adult Primary Care, Pediatric Primary Care, OB & Psychiatry.
  • May be required to oversee other clinical areas in the absence of a directors.
  • Provides ongoing mentorship and professional development of direct reports.

  • Onboards and orients direct reports to elements pertaining to leadership, clinical processes/policies/protocols, quality program and administration processes.
  • Establishes positive relationships with local community health community and attends Colorado Community Health Network (CCHN) functions
  • Responsible for fulfilling contract requirements for CHPA and CO Access membership, including but not limited to participating in quarterly check-ins, submission and review of quality data, participation in performance improvement projects, initiatives and deliverables required by members.
  • Participates in CU Nursing Clinical and Community Affairs (CCA) meetings and Health Care Partners Governing Board to stay apprised of changes at the CCA and to serve as a voice and advocate for Clinical sites.
  • Clinical champion for HRSA Operational Site Visit required to maintain FQHC status- responsible for implementing clinical process changes to ensure compliance with HRSA requirements.
  • Leads or participates on grant-funded projects that align with strategic plan and organizational action plan.

Quality:
  • Provides oversight for the Quality Improvement & Quality Assurance Program.
  • Participates on the interdisciplinary Committee on Quality Improvement (CQI) Meetings
  • Works in conjunction with Quality Director to process patient grievances, mortality reviews and clinical event reports to identify potential and actual quality of care incidents. Creates action plan and follow-through for all identified QOC incidents
  • Quarterly oversight of Uniform Data Set Measures to compare against internal goals as well as state and national benchmarks. Creates QI projects, identifies gaps and oversees focus PDSA cycles to ensure ongoing continuous quality improvement process
  • Works collaboratively with leadership team to achieve strategic goals. Responsible for operationalizing, monitoring and demonstrating outcomes of clinical aspects of strategic goals.
  • Serves as an effective team member; functions independently, exercises sound judgment and initiative; demonstrates flexibility to organizational priorities; maintains confidentiality; establishes and maintains effective interpersonal work relationships, and effectively assists providers.
  • Performs quality assurance and improvement activities; collects, analyzes and disseminates data; prepares reports; and demonstrates improvement
  • Prepares status reports for the Executive director, Associate Dean of the CCA and the Board of Directors for grant deliverables, QI Initiatives and other reports, as needed

Clinical Compliance:
  • Maintains current knowledge of best practice, standards of care and pertinent developments in fields of oversight. Develops, adapts and disseminates evidence-based clinical policies, procedures, and protocols on an ongoing basis.
  • Reviews all clinical policies and procedures every 3 years and as needed for updates, improvement, and relevance. Maintains a centralized database of clinically relevant and accessible policies & procedures for staff and faculty.
  • Serves as a clinical resource for best practice and standards of care for medical providers.
  • Maintains clinical expertise by upholding clinical practice at a portion of FTE
  • Provides ongoing, structured trainings and meetings for staff inclusive of integrated care meetings
  • Arranges pertinent trainings for clinical staff based on areas of interest or identified clinical gaps/needs
  • Leads NCQA PCMH Initiative annually to maintain accreditation and ensure compliance with standards
  • Works collaboratively with other clinical leadership in design and implementation of integrated, interprofessional models of care delivery and evaluation of patient outcomes
  • Supports and actively engages in cultural competency through Cultural and Linguistically Appropriate Services (CLAS) Standards
  • Assists with the development of reports for local, state, and federal authorities according regulatory requirements
  • Works closely with clinical pharmacy program to maintain robust integrated collaborative drug therapy management program for chronic disease management.

Academic Excellence:
  • Contributes to pertinent fields of research by participating in local and national conference on an ongoing basis to disseminate organizational research and quality improvement results
  • Identifies potential areas for research and publication related within the clinic and incorporates staff and faculty in the process of research, publication and presentation.
  • Coordinates graduate-level student placement in the fields of Adult-Gerontology Primary Care, Family Practice, Pediatric Primary Care, Women's Health, Psychiatry & Mental Health
  • Coordinates doctoral-level student placement in QI Projects identified as appropriate
  • Coordinates and oversees Bachelor of Science in Nursing (BSN) student placement for community health rotation, UCAN and immersion students.
  • Facilitates academic program development by identifying areas of expertise among SHS Employees and coordinating guest lectures to undergraduate and graduate programs in conjunction with Academic program directors at the CON.

Clinical:
  • Serves as primary care provider within team-based care model
  • Performs accurate physical examinations, evaluations, diagnoses, treatments, follow-up, and consultation
  • Supervises and/or coordinates the activities of patient care
  • Participates in after-hours call
  • Obtains patient history and develops comprehensive plan of care
  • Provides health education to patients, students, families, and/or community health practitioners and groups

Anticipated Pay Range:

The starting salary range (or hiring range) for this position has been established as HIRING RANGE:
  • $94,935 - $106,237 annually at Instructor of Clinical Practice/FNP/AGNP
  • $95,884 - $107,299 annually at Sr. Instructor of Clinical Practice/FNP/AGNP
  • $96,833 - $108,361 annually at Assistant Professor of Clinical Practice/FNP/AGNP

The above salary range (or hiring range) represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.

In addition to the annual salary for this position, an annual administrative stipend of $10,000/year will also be offered.
